Intending to be legally bound hereby and for the consideration of using the glass blowing studio (the “Activity”), the undersigned agrees to the following;

I am fully capable of entering into this agreement.

I understand and acknowledge that the use of the glass studio involves an inherently dangerous activity. I understand that Glass Blowing can lead to minor burns, cuts and bleeding. 

I agree to and hereby assume any and all risks associated with the Activity.

With full knowledge of the implications, I hereby release from liability and to indemnify and hold harmless, Eben Horton, Jennifer Nauck, The Glass Station and any of its employees or agents. 

This release is for any and all liability for any and all personal injuries (including death) which may occur while participating in the Activity and any property losses or damage occasioned by, or in connection with the Activity or accommodations for the Activity. 

I also further agree to abide by all the rules and regulations required to participate in the Activity.

I acknowledge that I fully understand the implication of this LIABILITY RELEASE FORM and enter into this agreement without reservation.
